When you contract care with Birthroot Midwifery, you will be given a Welcome Folder which contains information that will be helpful and needed throughout the course of your care.

 

To help save paper and reduce costs, the Informed Consent Documents are published to this website. Please read over them thoroughly and then indicate your preferences on the ROUTINE PRENATAL TESTING CONSENT form located in your folder.
